Description

AROGLIM-MP2 is an antidiabetic tablet that consists of Glimepiride 1 mg, Pioglitazone 15 mg and Metformin Hydrochloride 500 mg, marketed by Arrowin Pharmaceuticals in collaboration with its Medace Cardio-Diabetic brand division. This tri-fixture is one of the most scientifically advanced antidiabetic formulas and attacks three separate pathophysiological aspects of Type 2 diabetes: deficiency of insulin secretion (Glimepiride), insulin resistance in the periphery (Pioglitazone), and excess of glucose production in the liver (Metformin). As a result of attacking all three basic pathologies within one tablet, AROGLIM-MP2 offers superior multiple-action control of blood sugar levels compared to any dual-action combination formula and makes the medication scheme simpler for the patient.

AROGLIM-MP2 is manufactured under GMP-WHO certification and is distributed by Arrowin Pharmaceuticals’ PCD Pharma Franchise – a uniquely differentiating and clinically attractive tri-fixation for diabetologists and general practitioners treating complex and difficult-to-treat cases of Type 2 diabetes in India.

Uses and Indications

AROGLIM-MP2 tablets are recommended by diabetologists, endocrinologists, and physicians for the following indications:

  • Diabetes mellitus type 2 which is poorly controlled by two oral antidiabetic drugs (Glimepiride+Metformin, or Metformin+Pioglitazone) – for triple therapy
  • Insulin-resistant type 2 DM – Pioglitazone acts specifically on insulin resistance in adipose tissue and skeletal muscles
  • T2DM with metabolic syndrome – Pioglitazone corrects dyslipidemia, decreases visceral adiposity, has anti-inflammatory effect on vessels
  • Fatty liver disease (NAFLD/NASH) associated with type 2 DM – Pioglitazone is one of the few drugs with demonstrated efficacy in NASH
  • T2DM in patients with intolerance or insufficient response to high doses of monotherapy or dual therapy
  • PCOS complicated by insulin resistance and T2DM – both Pioglitazone and Metformin are used in PCOS
  • Simplified triple oral therapy in one pill

How Glimepiride 1 mg + Pioglitazone 15 mg + Metformin Hydrochloride 500 mg Tablets Works

AROGLIM-MP2 delivers triple-mechanism glycaemic control — the most comprehensive oral antidiabetic approach targeting all three primary defects of Type 2 diabetes simultaneously.

Glimepiride 1 mg is the insulin secretagogue, which binds to sulphonylurea receptor 1 (SUR1) on beta-cells of the pancreas, closes ATP-sensitive potassium channels, causes depolarisation of the cell membrane and calcium influx into the cell, which causes release of insulin. This 1 mg dose is the starting one — mild but effective, especially for triple therapy with Pioglitazone and Metformin, which provide considerable independent reduction of the blood glucose level, and thus require a lower sulphonylurea dose for reaching the target HbA1c.

The thiazolidinedione Pioglitazone 15 mg is the full agonist of peroxisome proliferator-activated receptor gamma (PPARγ), a nuclear transcription factor mainly expressed in adipose and skeletal muscles. PPARγ activation increases expression of genes coding glucose transporters (GLUT-4), fatty acid storage in subcutaneous adipose tissue and secretion of adiponectin. Clinically, there is an impressive improvement in insulin sensitivity in the periphery: muscle cells take up more glucose with respect to insulin, and adipose tissue becomes redistributed from the viscera to the subcutaneous fat depots. Pioglitazone decreases free fatty acid concentration and levels of inflammatory cytokines (TNF-α, IL-6) contributing to insulin resistance.

Metformin Hydrochloride 500 mg stimulates AMPK activity in the liver, thereby decreasing gluconeogenesis and glycogenolysis in the liver, resulting in reduced hepatic glucose production, which is the main cause of fasting hyperglycemia in T2DM. Metformin also increases peripheral muscle insulin sensitivity via an AMPK-mediated pathway, decreases glucose uptake from the intestines, and influences gut microbiota in a beneficial way. With 500 mg of Metformin, one can establish a tolerable base dose of hepatic glucose lowering as part of the triple combination.

Dosage and Administration

AROGLIM-MP2 must be taken orally, preferably with or right after the major meals according to the prescription from the diabetologist or physician. It is better to take it with meals since this helps avoid any Metformin-related stomach side-effects and also allows for proper insulin secretion time of Glimepiride due to its meal-related mode of action. Skipping meals after the medication is not recommended because Glimepiride causes the stimulation of insulin secretion irrespective of food consumption, thus leading to hypoglycaemia.

Side Effects

The side effects profile of Glimepiride 1 mg + Pioglitazone 15 mg + Metformin Hydrochloride 500 mg Tablets includes the side effects of all three components and includes the following:

  • Hypoglycaemia – due to glimepiride; less of a risk when 1 mg of glimepiride is used compared to sulphonylurea drugs
  • Fluid retention and oedema – most common side effect of pioglitazone; ankle swelling is commonly reported
  • Weight gain – as glimepiride and pioglitazone cause weight gain, but this side effect is minimised by metformin
  • Nausea, diarrhoea or abdominal discomfort – due to metformin; minimised when taken with food
  • Risk of heart failure – as pioglitazone leads to fluid retention; should not be used in case of heart failure
  • Risk of bone fractures – as pioglitazone increases the risk of bone fractures, especially in females; evaluate bone health of the patients on long-term therapy
  • Mild anaemia – due to dilutional anaemia caused by increase in the plasma volume of pioglitazone
  • Bladder cancer – associated with increased risk of bladder cancer with long-term use (>12 months) of pioglitazone; avoid in case of bladder cancer
  • Rarely, lactic acidosis – due to metformin; risk is minimal when kidneys work normally

Patients suffering from side effects should consult their doctors immediately.

Precautions and Warnings

  • Look out for signs of heart failure: oedema, sudden weight gain, and difficulty breathing; call your doctor immediately and stop using Pioglitazone
  • Liver function tests prior to and periodically during treatment – Pioglitazone is not for those with increased ALT (>2.5× ULN)
  • Never go without food after consuming this medication – hypoglycaemia risk associated with Glimepiride
  • Renal function (eGFR) prior to and periodically during treatment – Metformin is not prescribed for eGFR < 30 mL/min/1.73m²
  • Stop taking Metformin 48 hours before and after undergoing an iodinated contrast procedure
  • Look for signs of bone loss in women taking Metformin long term – increased fracture risk for Pioglitazone in women
  • Do not take for those with active bladder cancer or a previous history of bladder cancer – risk associated with Pioglitazone
  • Do not consume alcohol – increases the risk of both hypoglycaemia and lactic acidosis
  • Avoid in pregnancy or breast-feeding without specific specialist advice
  • Use with caution in elderly patients – higher risk of fluid retention, fractures, and hypoglycaemia
  • Store out of reach of children
