Glimepiride 4 mg + Metformin Hydrochloride 1000 mg Tablets is the maximum strength fixed-dose antidiabetic tablet of the AROGLIM-M FORTE family from Arrowin Pharmaceuticals, which consists of Glimepiride 4 mg and Metformin Hydrochloride 1000 mg. This particular drug is meant for Type 2 diabetes mellitus patients who have failed to achieve target glycaemic control (HbA1c < 7%) with smaller doses of Glimepiride (1 mg, 2 mg, and 3 mg), taken with Metformin, and who need a further intensification of their sulphonylurea dosage. How Glimepiride 4 mg + Metformin Hydrochloride 1000 mg Tablets Works
AROGLIM-M4 FORTE uses the same dual antidiabetic mechanism as AROGLIM-M3 FORTE but with a step-up in Glimepiride dose from 3 mg to 4 mg, providing proportionally greater pancreatic beta-cell stimulation and insulin secretory output.
Glimepiride 4 mg binds to sulphonylurea receptor 1 (SUR1) on beta-cell membranes in the pancreas, blocking the ATP-sensitive potassium channels and inducing insulin exocytosis. The 4 mg dose of Glimepiride is nearly maximal in terms of its pharmacological effect on insulin secretion, resulting in increased basal insulin secretion as compared to 3 mg, as well as helping control persistent hyperglycaemia due to fasting due to beta-cell secretory failure. Glimepiride further increases insulin sensitivity via increasing GLUT-4 in skeletal muscles and fat tissues, partially treating insulin resistance associated with T2DM.
Metformin Hydrochloride 1000 mg activates AMPK in hepatocytes, blocking hepatic gluconeogenesis and glycogenolysis, which results in reduced hepatic glucose production during the fasting state. The 1000 mg of Metformin provides the full effect and the required counterpart of insulin sensitiser to Glimepiride secretagogue in the treatment of T2DM.
Dosage and Administration
AROGLIM-M4 FORTE is for oral use only. Take once or twice daily with meals, as prescribed by a diabetologist or physician. The higher Glimepiride dose (4 mg) mandates consistent meal timing — never skip meals after taking this tablet. Monitor fasting blood glucose, postprandial glucose, and HbA1c as directed. Self-monitoring of blood glucose (SMBG) is particularly important at this dose level to detect hypoglycaemia early. Do not exceed the prescribed dose without physician authorisation.
Side Effects
AROGLIM-M4 FORTE has a relatively high risk of hypoglycaemia when compared to low doses of Glimepiride. The following are the documented side effects:
- Hypoglycaemia – the most important and dose-related side effect occurring at 4 mg Glimepiride; sweating, tremors, palpitations, confusion, and hunger can be used as early warning symptoms
- Nausea, vomiting, or diarrhoea – caused by Metformin; minimised significantly by taking with meals
- Abdominal distension or stomach upset – usually temporary
- Weight gain – more apparent with high sulphonylurea doses
- Dizziness or fatigue – could mean hypoglycaemia
- Metallic taste – benign side effect related to Metformin
- Lactic acidosis – very rare side effect related to Metformin

Precautions and Warnings
- Never omit meals after administering this medication; the risk of hypoglycaemia is highest one to three hours post-administration of the 4 mg Glimepiride
- Always carry fast-acting glucose (glucose tablets, fruit juice, sugar) for the treatment of hypoglycemia
- Regular monitoring of blood glucose is necessary; self-monitoring of blood glucose (SMBG) is highly recommended at this dose
- Check for eGFR/serum creatinine prior to starting treatment and periodically throughout treatment; Metformin is contraindicated when eGFR is less than 30 mL/min/1.73m²
- Do not take Metformin 48 hours before and after administration of iodinated contrast dye procedures
- Do not consume alcohol; it enhances hypoglycaemia from Glimepiride and raises the risk of lactic acidosis with Metformin
- Take with extreme caution in elderly patients; greater chance of developing hypoglycaemia and impaired renal elimination
- Let your prescribing doctor know about any medications being taken simultaneously; NSAIDs, beta blockers, and fibrates potentiate hypoglycaemia
- Not suitable for pregnant and breastfeeding women without specific instruction from a specialist
- Keep away from children
Contraindications
AROGLIM-M4 FORTE is contraindicated in Type 1 diabetes mellitus, diabetic ketoacidosis, known hypersensitivity to Glimepiride, Metformin, or any sulphonylurea. Contraindicated in significant renal impairment (eGFR < 30 mL/min/1.73m²), severe hepatic impairment, and conditions predisposing to lactic acidosis. Contraindicated in pregnancy and breastfeeding.
