Description
Arofast CV-20 is a capsule containing a combination of Atorvastatin and Aspirin, marketed by Arrowin Pharmaceuticals Company under its Medace Cardio-Diabetic branch. A clinically logical fixed-dose combination of two main cardiovascular drugs — a statin for the management of lipids and an antithrombotic drug for the prevention of thrombosis — is presented in the Arofast CV-20 capsule. Both treatment of dyslipidemia with the help of statins and treatment of thrombotic risk with the help of Aspirin are independently indicated in case of increased risk of cardiovascular diseases according to international guidelines of cardiologists; thus, Arofast CV-20 capsule unifies them both in one capsule, increasing treatment compliance.
Arofast CV-20 capsule is produced under conditions of GMP-WHO certification and can be found in the portfolio of Arrowin Pharmaceuticals’ Medace Cardio-Diabetic PCD Pharma Franchise — a valuable combination for cardiologists, diabetologists and general practitioners of India.
Uses and Indications
Arofast CV-20 Capsules are prescribed by cardiologists, diabetologists, and general physicians for:
- Secondary prevention of cardiovascular disease – reduction in the risk of recurring myocardial infarction and ischaemic stroke among CAD patients
- Primary prevention of cardiovascular disease among high-risk patients – patients suffering from diabetes, multiple cardiovascular risks or having calculated 10-year cardiovascular risk beyond a certain limit
- Management of patients after a myocardial infarction episode – providing dual cardioprotection with lipid-lowering and antiplatelet properties
- Hypercholesterolaemia along with requirement of antiplatelet drug – consolidation of dual medication to be taken in form of single capsule
- Type 2 diabetes mellitus along with dyslipidaemia and cardiovascular risk – both drugs are recommended by guidelines in this category of patients
- Atherosclerotic cardiovascular disease (ASCVD) – secondary prevention and plaque stabilisation over the long run
- Patients undergoing PCI (percutaneous coronary intervention) needing statins along with antiplatelet medication
How Atorvastatin and Aspirin Capsules Works
Arofast CV-20 provides a combination of two complementary cardioprotective effects. Atorvastatin competitively blocks the HMG-CoA reductase enzyme, which catalyses the conversion of 3-hydroxy-3-methylglutaryl-coenzyme A to mevalonate in liver cells, hence lowering hepatic synthesis of cholesterol, causing an increase in the number of LDL receptors. It leads to a major decrease in plasma levels of LDL cholesterol (a causative factor of atherosclerotic plaque formation), total cholesterol and VLDL cholesterol, and a variety of other effects like improved endothelial function, anti-inflammatory effects, and atherosclerotic plaque stabilisation.
Aspirin is a prostaglandin endoperoxide synthase 1 inhibitor and causes irreversible inhibition of COX-1 in platelets, leading to irreversible inhibition of the production of the strong platelet activator and vasoconstrictor – thromboxane A2. The irreversible inhibition happens because platelets lack a nucleus and hence cannot synthesise COX-1. Thus, one dose of low-dose Aspirin irreversibly inhibits platelet aggregation for its entire life span of 8-10 days, preventing any thrombus formation on ruptured atheromatous plaques that lead to most cases of acute coronary syndromes and ischaemic strokes.
Dosage and Administration
Arofast CV-20 can be administered orally. One capsule should be consumed once daily at the same time daily with or without food, according to the directions from your cardiologist or doctor. Crush or break the capsule only if instructed. Grapefruit and its juice should be avoided while on Atorvastatin treatment. Liver functions and lipid levels need to be monitored as per your doctor’s orders. Both Atorvastatin and Aspirin have to be used lifelong for cardiovascular conditions.
Side Effects
Arofast CV-20 is generally well tolerated at recommended doses. Reported side effects from both components include:
- Gastric irritation, dyspepsia, or epigastric pain (due to Aspirin) — Take along with food to reduce
- Nausea or mild abdominal pain
- Myalgia (muscle pain/aches) (due to Atorvastatin) — Report if there is any persistent unexplained muscle pain
- Headache or dizziness
- Elevation of liver enzymes (due to Atorvastatin) — Monitor periodically
- Prolonged bleeding time (antiplatelet action of Aspirin) — Caution before any surgery
- Elevation of blood glucose/HbA1c (class effect of the statins)
- Rarely, gastrointestinal bleeding (due to Aspirin) — More common in patients with GI risk factors
- Rarely, myopathy/rhabdomyolysis (due to statins) — Higher risk when combined with interacting drugs
- Patients having persistent or severe side effects should see their physician immediately.
Precautions and Warnings
- Take with or after food to minimise Aspirin-related gastric irritation
- Inform the prescribing doctor of any history of peptic ulcer, GI bleeding, or gastric sensitivity before use
- Report any unexplained muscle pain, weakness, or dark urine immediately — risk of myopathy or rhabdomyolysis
- Avoid grapefruit and grapefruit juice — significantly increases Atorvastatin plasma levels
- Monitor liver function tests at baseline and periodically during therapy
- Inform the surgeon or dentist of Aspirin use before any surgical or dental procedure — affects bleeding
- Do not take additional NSAIDs, anticoagulants, or other antiplatelet drugs without physician advice — additive bleeding risk
- Absolutely contraindicated in pregnancy — Atorvastatin is teratogenic
- Use with caution in patients with asthma — Aspirin may trigger aspirin-sensitive asthma in some individuals
- Keep out of reach of children
Contraindications
Arofast CV-20 is absolutely contraindicated in pregnancy and breastfeeding (Atorvastatin is Category X — teratogenic). Contraindicated in known hypersensitivity to Atorvastatin, Aspirin, or any NSAID. Contraindicated in active peptic ulcer disease, severe hepatic impairment, and aspirin-sensitive asthma. Avoid in patients with active gastrointestinal bleeding.
Storage Instructions
Store below 25°C in a cool and dry place, protected from light and moisture. Keep in original packaging and out of reach of children.
