The third-generation sulphonylurea glimepiride 3 mg binds to sulphonylurea receptors (SUR1) at the membrane of pancreatic beta cells. It closes ATP-sensitive potassium channels (KATP), leading to membrane depolarisation, opening of voltage-dependent calcium channels, an increase in intracellular calcium and, thus, insulin secretion. Unlike previous generations of sulphonylurea drugs, glimepiride has greater specificity to pancreas SUR1 rather than cardioprotective preconditioning interfering with SUR2A. In addition, this drug increases insulin sensitivity in peripheral tissues by augmenting the quantity and function of GLUT-4 glucose transporters.
The primary mechanism of metformin hydrochloride 1000 mg action is related to the activation of AMP-activated protein kinase (AMPK) in hepatocytes — an energy-sensing enzyme that inhibits gluconeogenesis and glycogenolysis in the liver cells and lowers hepatic glucose output up to 30% during fasting. Also, metformin enhances insulin sensitivity in peripheral tissues (mainly in skeletal muscle cells), inhibits intestinal glucose absorption and has some positive impact on the microbiome of the gut and on bile acid metabolism. Dosage and Administration

Side Effects
AROGLIM-M3 FORTE is generally well tolerated at recommended doses. Reported side effects include:
- Hypoglycaemia (low blood sugar) — the most clinically significant risk; related to the Glimepiride component; more likely if meals are skipped or physical activity is significantly increased
- Nausea, vomiting, or diarrhoea — most common with Metformin, especially during initiation; take with food to minimise
- Abdominal bloating or gastric discomfort — transient; usually resolves within 1–2 weeks of treatment initiation
- Metallic taste in the mouth — a benign Metformin-related effect
- Dizziness, sweating, or tremors — signs of hypoglycaemia; treat immediately with fast-acting glucose
- Mild weight gain — possible with Glimepiride component; less pronounced than with older sulphonylureas
- Very rarely, lactic acidosis — a serious but rare Metformin complication; risk is extremely low in patients with normal renal function

Precautions and Warnings
- Monitor blood sugar closely, especially during the first few weeks of treatment – high Glimepiride levels predispose to hypoglycaemia.
- Never miss a meal following the ingestion of this medication – the sulphonylurea portion induces the release of insulin irrespective of eating
- Renal function tests (serum creatinine / estimated GFR) must be done before starting the drug and while on therapy – Metformin is contraindicated in patients whose eGFR is less than 30 mL/min/1.73m2
- Stop Metformin 48 hours before and after using iodinated contrast dye – risk of contrast-induced acute kidney injury and lactic acidosis
- Avoid alcohol – enhances hypoglycaemic action of Glimepiride and predisposes to lactic acidosis with Metformin
- Notify the prescribing physician regarding all current medications – NSAIDs, beta blockers, and other anti-diabetics may interact
- Exercise caution when administering to patients with hepatic dysfunction – Metformin and Glimepiride are relatively contraindicated in significant liver disease
- Advise the patients to always have glucose tablets and fruit juice handy to counteract hypoglycaemia attacks
- Do not take oral anti-diabetics in pregnancy and lactation period without proper guidance from a specialist
- Keep out of reach of children
Contraindications
AROGLIM-M3 FORTE is contraindicated in Type 1 diabetes mellitus and diabetic ketoacidosis. Contraindicated in known hypersensitivity to Glimepiride, Metformin, or any sulphonylurea. Contraindicated in significant renal impairment (eGFR < 30 mL/min/1.73m²), severe hepatic impairment, and conditions predisposing to lactic acidosis (acute MI, respiratory failure, sepsis, dehydration).
