EMWIN-M

It is a combination antidiabetic tablet by Arrowin Pharmaceuticals, available in two strengths: Emwin-M 5/500, containing Empagliflozin 5mg and Metformin 500mg, and Emwin-M 12.5/500, containing Empagliflozin 12.5mg and Metformin 500mg. It is prescribed by diabetologists, endocrinologists and physicians managing Type 2 diabetes where dual-mechanism glucose control is indicated, particularly in patients who also carry cardiovascular or renal risk factors.

Metformin is the established first-line oral antidiabetic drug for the management of Type 2 diabetes and has been so for decades. It works primarily by reducing hepatic glucose production and improving peripheral insulin sensitivity, lowering fasting blood glucose levels effectively without causing hypoglycaemia when used as monotherapy. Its long safety record, weight-neutral profile and affordability make it the anchor of most Type 2 diabetes treatment regimens.

Empagliflozin belongs to the SGLT-2 inhibitor class, one of the most significant advances in diabetes pharmacology in recent years. It works by blocking sodium-glucose cotransporter-2 in the kidneys, which reabsorbs approximately 90% of filtered glucose back into the bloodstream. By inhibiting this transporter, Empagliflozin causes excess glucose to be excreted in the urine, thereby reducing blood glucose levels through a mechanism entirely independent of insulin. This is a meaningful clinical advantage because it works regardless of the patient’s residual beta-cell function or degree of insulin resistance.

What sets Empagliflozin apart from older antidiabetic agents is its demonstrated cardiovascular and renal protective effects beyond glucose control. Clinical evidence has shown that Empagliflozin significantly reduces the risk of hospitalisation for heart failure, slows the progression of chronic kidney disease and reduces cardiovascular mortality in patients with established cardiovascular disease. For the diabetologist managing a patient with both Type 2 diabetes and cardiac or renal comorbidities, Empagliflozin is no longer just a glucose-lowering option — it is a cardio-renal protective agent that also lowers glucose.

The combination of Empagliflozin and Metformin in Emwin-M provides complementary glucose-lowering effects through two independent pathways, resulting in better overall glycaemic control than either agent alone at equivalent doses. The 5/500 strength is appropriate for initiating SGLT-2 inhibitor therapy in patients already on Metformin, while the 12.5/500 strength is appropriate for patients requiring higher Empagliflozin doses for more robust glucose and cardiovascular risk management.

Composition

Emwin-M 5/500: Each tablet contains Empagliflozin IP 5mg and Metformin Hydrochloride IP 500mg.
Emwin-M 12.5/500: Each tablet contains Empagliflozin IP 12.5mg and Metformin Hydrochloride IP 500mg.

Therapeutic Class

SGLT-2 Inhibitor and Biguanide Antidiabetic Agent.

Indications

Emwin-M is indicated as an adjunct to diet and exercise to improve glycaemic control in adults with Type 2 diabetes mellitus. It is particularly appropriate for patients with established cardiovascular disease or at high cardiovascular risk, patients with chronic kidney disease requiring renoprotective antidiabetic therapy, and patients inadequately controlled on Metformin alone who require additional glucose-lowering with cardiovascular benefit.

Dosage and Administration

Emwin-M should be taken orally with meals to reduce gastrointestinal side effects from the Metformin component. The starting strength and dose are determined by the treating physician based on the patient’s current antidiabetic regimen, renal function and cardiovascular status. Do not initiate Empagliflozin-containing therapy in patients with an eGFR below 30 ml/min/1.73m². Always follow the prescribing physician’s instructions on dose, timing and monitoring requirements.

Packaging

10×10 ALU-ALU blister pack.

Are There Any Side Effects of Emwin-M?

Itis generally well tolerated. The most commonly reported side effects relate to the individual components. Metformin may cause gastrointestinal effects including nausea, diarrhoea and abdominal discomfort, particularly at initiation, which are minimised by taking the tablet with food and starting at a lower dose.

Empagliflozin increases urinary glucose excretion, which raises the risk of urinary tract infections and genital mycotic infections, particularly in female patients. Patients should be counselled on adequate hydration and genital hygiene during therapy. Volume depletion and mild diuresis may occur, which is relevant in elderly patients or those on concurrent diuretic therapy.

FAQ Section

What is Emwin-M used for?

Emwin-M is an antidiabetic tablet combining Empagliflozin and Metformin for the management of Type 2 diabetes. It is prescribed to improve blood sugar control through two independent mechanisms and is particularly suited to patients with cardiovascular disease, heart failure risk or chronic kidney disease who need antidiabetic therapy with organ-protective benefits.

What is the difference between Emwin-M 5/500 and Emwin-M 12.5/500?

Both contain Metformin 500mg and differ in the Empagliflozin dose. Emwin-M 5/500 is typically used to initiate SGLT-2 inhibitor therapy in patients already on Metformin. Whereas Emwin-M 12.5/500 provides a higher Empagliflozin dose for patients requiring stronger glucose-lowering or greater cardiovascular protection. The treating physician determines the appropriate strength based on clinical assessment.

Why is Empagliflozin preferred in diabetic patients with heart disease?

Empagliflozin has demonstrated in large clinical trials that it reduces hospitalisation for heart failure and cardiovascular mortality in patients with Type 2 diabetes and established cardiovascular disease. This makes it more than just a glucose-lowering drug. For patients who carry both diabetic and cardiac risk, adding Empagliflozin to a Metformin regimen addresses both concerns simultaneously.

Can Emwin-M cause low blood sugar?

Metformin and Empagliflozin individually do not cause hypoglycaemia when used without other antidiabetic agents such as insulin or sulfonylureas. However, when Emwin-M is used as part of a combination regimen with insulin or sulfonylureas, the risk of hypoglycaemia increases and dose adjustment of the other agents may be required under physician guidance.
